PCOS is a genetic disease and mainstay of the treatment is weight reduction. For PCOS I am using PCO Care Tablets. Does it work?

For PCOD or PCOS, PCO Care Tablet are of no use. You just need to control your weight. Even if your BMI is normal, you should jog or brisk walk daily to maintain normal weight or if you are overweight then reduce weight to bring it down to BMI of <25. Menses will be irregular with PCOD, even with treatment, menses will not come back to normal unless weight is controlled.

PCO Care Tablet contains Myo-Inositol, which is a naturally occurring substance belonging to the B complex family of vitamins. Research till now suggest that myoinositol may be useful in the treatment of PCOS patients undergoing ovulation induction, both for its insulin-sensitizing activity, and its role in oocyte maturation. Recently published Cochrane article (highest quality of evidence in 2013) concluded that any Antioxidants like Myo-inositol are not associated with an increased live birth rate or clinical pregnancy rate. So it is better to avoid such kind of drugs.

How to bring menses in PCOD? How to use Tablet Deviry for getting menses in PCOD?

Tab Deviry is used in the dosage of 10 mg twice a day for 5 days. Bleeding will start within 7 days once you stop taking Deviry after 5 days. You should wait for at least 3 months to get your spontaneous menses without taking medicines. Primary objective of your treatment still remains weight reduction and maintain normal BMI of <25.

Formula for calculating BMI is Weight in Kg divided by Height in meter square. Primary objective of your treatment is weight reduction and maintain normal BMI of 18-25. This will regularize the monthly cycles and also help you get pregnant. You can take Metformin to reduce weight. You don’t need to take any medicine till 3 months for the menses to come. Only after 3 months of missed menses you should take medicines like progesterone Tablet (Deviry 10 mg twice a day for 5 days) for getting menses. For reducing your weight you should have brisk walk or jogging for 4-5 km daily, high protein diet (fish, chicken, mutton without fat, pulses, mushrooms), low carbohydrate diet (avoid rice, chapatti, bread, poha, upma, idli, dosa, potato, sugar etc and can use bhakri instead) and low fat i. e. less oil and preferably use rice bran oil.

I am suffering from PCOD problem and planning to get pregnant. Should I continue Glycomet? My weight is normal.

Glycomet or Metformin is started in PCOD patients who are overweight. As u are already normal weight, there is no benefit of taking Glycomet. It should be stopped immediately as you have normal weight and it is not started normal weight with PCOD. In the meantime you should start taking folic acid Tablets as you are planning to get pregnant and maintain normal weight with Jogging or brisk walk for 4-5 km daily.

I am a PCOD patient with repeated Vulva infection. How to get relief?

If you have frequent vulva infection it is because of fungal infection. This is a common symptom of diabetes, which goes hand in hand with PCOD. Please check your sugar to rule out diabetes. If you are overweight, then you need to reduce your weight to control symptoms of PCOD and ultimately sugar.

I have PCOD problem. We are planning for pregnancy. I want to know the treatment to get pregnant in PCOD.

You have to try for one full year for getting pregnant. If there is no pregnancy after 1 full year then you have to start taking treatment. In PCOD, to get pregnant, clomiphene citrate to be started from day 3 of menstrual cycle and egg growth to be monitored by follicular scan. This is the normal course of treatment initially for any woman with infertility with PCOD. As you are suffering from PCOD, menses are likely to be irregular and scanty. There is no need to take medicines to get your regular as it will be same story again next month once you stop medicines. Krimson is given, only if patient has hirsutism or excessive hair. As for menses are concerned, there is no need to panic, wait for another 3 month for the menses to come. I am suffering from PCOD and there is acne. How to get relief from acne?

Retinoids, and antibiotics may be used to treat acne. These products may be available in pills, creams, or gels. The specific treatment depends on the severity of the acne and how long it has been visible. Because retinoids can cause birth defects, you should not use them if you want to become pregnant.

I have PCOD problem and taking Krimson-35, is their chances of pregnancy?

While you are taking Krimson, you cannot become pregnant as these are oral contraceptive pills, which prevents pregnancy. If you have normal weight, so there is no need to worry. First line of treatment PCOD is weight reduction if you are overweight with BMI >25. Krimson is started when patient has excessive hair growth, it will stop new hair growth. If hair growth is not, then there is benefit of taking Krimson. You do not need to treat irregular menses, if you are planning for pregnancy. If you have already tried to get pregnant for 1 full year, then Clomiphene Citrate should be started along with follicular monitoring to get pregnant.

I have PCOD hormone problem from last 2 year. What should I do? My weight is normal?

I am having PCOD. Am taking Ovral pills for regularizing cycles. Will it work?

Ovral pills/ Femilon/ Novelon/ Duoluton all are oral contraceptive pills and they are never prescribed for PCOD. There is no point in regularizing menses with Ovral as it will not get cured with it and next month it will be same. Reducing weight only can control PCOD and will increase your chance to get pregnant. Diet along with exercise and Metformin will help in reducing your weight and ultimately getting pregnant. Krimson or Ginette are the options for you, if you are suffering from excessive hair growth, during this time meses will also come regular.

PCOD and Menses Problem
 I am 18 year-old girl and my periods are irregular and it comes after 3 month 4 month. I am not overweight. How to ensure a normal period when you are suffering from PCOS?

You are likely to be suffering from PCOD. Please get an ultrasound done along with TSH, FSH and LH and Prolactin blood test. You are likely to be suffering from PCOD or thyroid problem. Breast development/growth stops between ages 17-21 years. When the ovaries start to secrete estrogen breasts enlarge at puberty. The rate at which breasts growth is different in each young woman. Breasts actually never stop growing till the age of 45 years when menopause sets in. Breast size will not increase after 20 years unless you put on weight only if you are underweight.

I am married recently and having breast pain while touching.

During menses, hormones can cause changes in breast tissue that can cause pain or discomfort. Breast pain is called mastalgia, and is a common problem. Pain associated with the menstrual cycle gets relieved automatically after the menses are over.

Noncyclical pain can be because of injury or excessive massage to the breast. It may not be related to the breast at all, and it might be originating from the underlying muscle. While pain which is not related to menses avoid touching and manipulation by husband or partner for some period. In the meantime you can take pain killers and avoid using tight bra.

If the localised breast pain persisting for a month with no change, then definitely you need to report to the doctor who after breast examination may suggest or advise mammogram or sonography or needle biopsy.

I am 16 years of age and suffering from breast pain. What is the treatment?

As you are only 16 years old, there is no chance that you r suffering from any major disease. It is common among your age group and is caused by hormonal changes. It will go after some time. In the mean time you should take pain killers.

I have guilty in breast which gives pain, FNAC and ultrasound done and reports are normal.

As your FNAC and ultrasound test are normal and If there are 4 Knots or Guilty which you can easily feel and count also, then it has to be fibroadenoma, which should be removed surgically as you have pain there. Fibroadenomas are not cancer and don’t increase the risk of developing breast cancer. They occur because of an increased sensitivity to the female hormone estrogen.

I am overweight and want to reduce both weight and breast size also.

I have big lump on my left breast which is not painful. Do I need to get surgery?

It looks like it is fibroadenoma, it should be removed after initial investigation like mammography and sonography of the breast and/or FNAC if it is painful. Fibroadenomas are not cancer and don’t increase the risk of developing breast cancer. They occur because of an increased sensitivity to the female hormone estrogen.

A fibroadenoma usually has a smooth rubbery feel and can move easily under the skin. They are usually painless, but some people may feel pain. It is very common can occur in numbers. It can occur at any age. Most are about 1 to 3cm in size but occasionally, it can grow to more than 5cm. Most fibroadenomas stay the same size. Some get smaller and some eventually disappear over time. Sometimes surgery is required to remove large fibroadenomas.

I have breast lump

When to see a doctor? I have breast lump.

Around >90% of breast lumps are benign means not cancer. You also should see a doctor if you notice any of the following changes:

  • Thickened tissue on breast of firm feel of the skin
  • Nipple discharge, with or without blood
  • Change in the size or shape of your breasts
  • Dimpling on the skin of breasts like Orange
  • Rash on or around your nipples
  • Nipple appearance changes like sunken into breast
  • Pain in your breasts or armpits
  • Lump or swelling in armpits
I am 46-years-ol. I am feeling pain in my left breast. It is not regular pain. What should I do?

‘Triple Assessment’ is done for definite diagnosis which consists of: breast examination, mammogram (breast x-ray) or ultrasound scan, and/or a fine needle aspiration (FNA) or core biopsy.

Mammogram identifies how serious is the abnormality that was found. This and other information is efficiently summed up in one number, called the Breast Imaging Reporting and Data System (BI-RADS) score.

BI-RADS scores range from 0 to 6; based on which your future treatment will depend.

Score 1 and 2 is good news! It means that your mammogram is negative. Score 3 means that mammogram is probably normal but that there’s an approximately 2% chance of cancer. Score 4 means that the findings are suspicious and there is an approx. 20% – 35% chance that a breast cancer is present. To make a diagnosis, biopsy will be performed. Score 5 means that your mammogram results are highly suspicious, with a 95% chance of breast cancer. Score 6 means that you have already been diagnosed with breast cancer and the pathologist has confirmed the diagnosis.

I have Breast Lump which is Red and Tender and paining a lot. All surrounding breast has become red. What am I suffering?

If the lump is red and tender it is most likely breast abscess. Small breast abscess is with the needle without giving incision. If the abscess is large, small incision is needed to drain the pus. Along with these two procedures oral antibiotics will be given for 5 days. It can be done under local anesthesia.

Breast abscess is a collection of pus in one part of the breast. It develop are caused by infection. They are painful and lumps will be red, hot, with swelling and can cause fever. It usually affects breastfeeding mothers if milk ducts gets blocked.

I have a lump since 3 years and mammography is normal. What should I do? What is the treatment of fibroadenosis of breast? I have swelling and pain in one breasts which increases during menses. What am I suffering from?

You should get sonography of the breast as well as mammography of the breast done to have clear idea about the lump. Mammography may not pick up the lump. It is likely to be fibroadenosis which is related to hormonal changes. It is a common non- cancerous breast condition. In most cases treatment is not needed for fibroadenosis or fibrocystic breast changes. Removing fluid through needle aspiration may help relieve symptoms of pain or pressure temporarily.

To reduce discomfort treatment is : Supportive bras, Pain killers like Brufen or Voveran, Reduced intake of caffeine found in coffee, tea and chocolate.

Oral contraceptives for 3-6 months like Femilon or Novelon is a good idea for women with severe symptoms. They are to be started from any day from Day1 – Day5 of menses.

Is it safe to terminate pregnancy at 6 weeks?

It is most safe to terminate pregnancy early on. Abortion pills are most effective before 7 weeks. Mifepristone and misoprostol pills are used in combination. However, it can also be performed surgically using vacuum aspiration and/or evacuation.

Continuous bleeding for 1-month after abortion pills.?

Bleeding for one months after abortion pills is not normal. Bleeding can occur from 4-14 days from the day of taking the pills. If bleeding still persists after 14 days, gynaecologist needs to be consulted immediately to confirm the completeness of abortion. Ultrasound will confirm if any products are remaining in the uterus. Retained products needed to be removed surgically to prevent further complications like infection and infertility.
